Tracking alcohol-related incidents may be one of the most important form of documentation completed on event day. Recording all alcohol-related incidents can help the facility track trends as well as improve venue management of alcohol-related problems.

The information below shows highlights when incident report forms should be completed.

When to Document:
  • Physical harm
  • Property damage
  • Code of Conduct violation
  • Underage drinking
  • Underage attempt to purchase alcohol
  • Fake ID
  • Outside alcohol found inside
What to Document:
  • Description of the incident(what happened, time, date, location) and guest(s) involved
  • Information about steps taken and final outcome
  • List of witnesses and employees and/or managers involved
  • Clarify witness statement from second-hand accounts
What NOT to include:
  • Any opinions of a guest or the incident
  • Admission of fault or guilt by employee or facility
